Most common with PS3 / DualShock 3 pads, which have no native Windows driver: install DsHidMini (Windows) or run the pad through Steam with PlayStation Configuration Support enabled, then reload this page.
Also try: a different USB cable (some are charge-only), a different USB port, or pressing the PS/Home button.
